صفحه 1زمینه و هدفتعداد زیادی از زنان بعد از زایمان با برش اپی زیاتومی درگیر عوارض کوتاه مدت می شوند و بیش از 20 % آنها عوارض طولانی مدت ناشی از این برش را تجربه می کنند. عواملی مانند نوع نخ به کار رفته، روش ترمیم زخم و مهارت جراح بر میزان عوارض اپی زیاتومی تاثیر می گذارند. این پژوهش با هدف مقایسه بهبود زخم اپی زیاتومی در زنان زائو توسط نخ های کات گوت کرومیک و ویکریل طراحی گردید.مواد و روش هاپژوهش حاضر مطالعه ای کارآزمایی بالینی تصادفی شاهدار بود. نمونه پژوهش 102 زنان زائو بودند که به طور تصادفی به دو گروه 51 نفری ترمیم اپی زیاتومی با نخ کرومیک و ترمیم با نخ ویکریل تقسیم شدند. دو گروه در 4-6 هفته بعد از زایمان از نظر ترمیم بافتی اپیزیاتومی ترمیم بررسی شدند. ترمیم بافتی با استفاده از مقیاس ریدا مورد سنجش قرار گرفت. داده های بدست آمده با استفاده از آزمون های آماری تی تست، من ویتنی، مورد تجزیه و تحلیل قرار گرفتند.یافته هادو گروه از نظر متغیرهای سن، سن حاملگی، تعداد زایمان، طول مرحله دوم ایمان، فاصله بین زایمان و ترمیم اپی زیاتومی، مدت زمان ترمیم اپی زیاتومی و میزان تحصیلات اختلاف آماری معنی داری نداشتند. میانگین مقیاس ریدا در گروه ترمیمی با نخ ویکریل به طور معنی داری کمتر از گروه کرومیک بود(05/0>P). همچنین میانگین نمرات قرمزی محل ترمیم، ادم، اکیموز و خروج ترشح از زخم در گروه ویکریل به طور معنی داری کمتر از گروه کاتگوت بود اما در آیتم نزدیکی لبه های زخم اختلاف آماری معنی داری بین دو گروه مشاهده نشد.نتیجه گیریبا توجه به نتایج بدست آمده از پژوهش حاضر نخ ویکریل در مقایسه با نخ کرومیک جهت ترمیم اپیزیوومی دارای عوارض کمتری بوده و ارجح تر می باشد.کلیدواژگان: اپی زیاتومی، ویکریل، کاتکوت، بهبود زخم

صفحه 7زمینه و هدفدفتر مدیترانه شرقی سازمان جهانی بهداشت در برنامه بیمارستان های دوستدار ایمنی بیمار، خون و فرآورده های خونی ایمن را به عنوان یک استاندارد الزامی مورد توجه قرار داده است. هدف از انجام این پژوهش بررسی کیفیت آماده سازی خون و کیفیت پایش بیمار در زمان تزریق پس از استقرار استانداردهای ایمنی بیمار بود.مواد و روش هاپژوهش حاضر بر روی یک گروه و در دو نوبت متوالی در بیمارستان بهارلو، شهر تهران انجام گردید. به منظور انجام این مطالعه، داده های مربوط به سال 1391 (اوایل استقرار استانداردها) با سال 1393 (یعنی چهار سال بعد از استقرار استاندارد ها) مقایسه گردید. تعداد نمونه ها 100 عدد کیسه خون بود که توسط فرمول کاکران تخمین زده شده بود. داده ها توسط چک لیست جمع آوری گردید. تحلیل داده ها با استفاده از شاخص های آماری توصیفی و آزمون تی زوجی انجام شد.یافته هامیانگین شاخص کیفیت آماده سازی خون (299/0± 92/6) و پایش بیمار (654/0±96/7) بعد از استقرار استانداردهای ایمنی بیمار نسبت به اوایل استقرار استانداردها (به ترتیب482/0 ± 64/1و 611/0± 52/2) افزایش نشان داد و این افزایش با استفاده از آزمون تی زوجی برای هر دو شاخص از لحاظ آماری معنی دار بود (01/0 >P).نتیجه گیریاین پژوهش نشان داد که استقرار استانداردهای ایمنی بیمار، می تواند بر کیفیت آماده سازی خون و کیفیت پایش بیمار در زمان تزریق در نظام مراقبت از خون تاثیر چشمگیر داشته باشد.کلیدواژگان: تزریق خون، ایمنی بیمار، پایش بیمار

صفحه 15زمینه و هدفآموزش بالینی یکی از ارکان مهم آموزش پرستاری محسوب می شود. شایستگی فرهنگی به عنوان یکی از پیامدهای آموزش بالینی، موضوع مهم در آموزش پزشکی و پرستاری است که به واسطه آن، ارایه دهندگان مراقبت توانایی کارکردن به طور موثر را پیدا می کنند. شایستگی فرهنگی از طریق اجرا و به کارگیری هدفمند برخی راهبردها و فرایندهای یادگیری به دست می آید. هدف از نگارش مقاله حاضر، مروری بر رویکردهای نوین تدریس است که در آموزش بالینی دانشجویان پرستاری به کار گرفته شده است تا از میزان دستیابی به پیامدهای یادگیری در حیطه های مختلف بویژه حیطه شایستگی فرهنگی آگاهی کسب شود.مواد و روش هامطالعه مروری حاضر بر اساس جستجو در پایگاه های اطلاعاتی فارسی شامل Magiran، Iran medex، SID، Irandoc و با استفاده از کلید واژه های آموزش بالینی، آموزش بالینی در پرستاری، روش های نوین تدریس، روش های نوین تدریس در آموزش بالینی صورت گرفت. در مجموع 62 مقاله ایرانی استخراج شد که پس از مطالعه و تحلیل، 26 مقاله مرتبط انتخاب گردید.یافته هااز میان 26 مطالعه مرور شده، نقشه مفهومی و همکار آموزش بالینی هر کدام 6 مطالعه، انواع رویکرد های رایزنی 4 مقاله، رویکرد مبتنی بر کامپیوتر 2 مطالعه و رویکرد تلفیقی 3 مطالعه و بقیه سایر رویکرد ها را در بر گرفت.نتیجه گیریمرور مطالعات رویکردهای نوین تدریس در آموزش یالینی پرستاری ایران نشان داد که شایستگی بالینی عنصر گمشده آموزش بالینی پرستاری است. با توجه به اهمیت و ضرورت آن توصیه می شود که در رویکردهای آموزش بالینی بخصوص رویکردهای نوین مورد توجه قرار گرفته و از مطالعات کمی و کیفی برای چگونگی سنجش و ارتقا آموزش این عنصر مهم استفاده شود.کلیدواژگان: آموزش، پرستاری، شایستگی فرهنگی

صفحه 27زمینه و هدفآسیب ناشی از اجسام نوک تیز سبب ایجاد نگرانی، انجام آزمایش ها و دارو درمانی می شود. هدف این مطالعه بررسی فراوانی و عوامل مرتبط با آسیب ناشی از اجسام نوک تیز در بین دانشجویان پزشکی و پرستاری بود.مواد و روش هاطی یک مطالعه مقطعی، اطلاعات توسط پرسشنامه از 150 دانشجوی پرستاری و پزشکی دانشگاه علوم پزشکی شهید بهشتی جمع آوری شد که شامل اطلاعات دموگرافیک، دفعات، محل و علت آسیب بود. شاخص روایی محتوایی و پایایی پرسشنامه به ترتیب 83/0 و 78/0 برآورد گردید. اطلاعات با استفاده از SPSS ویرایش 16 و با استفاده از آزمون مربع کای و دقیق فیشر تحلیل شد.یافته ها3/41% از دانشجویان، حداقل یکبار دچار آسیب شده بودند. شیوع آسیب در بین دانشجویان پزشکی 4/48 درصد و در دانشجویان پرستاری 3/28 درصد بود. اطلاعات قبلی برای پیشگیری در دانشجویان پزشکی 66 درصد و پرستاری 100 درصد بود. بیشترین آسیب ها در بخش اورژانس و داخلی، بیشترین وسیله منجر به آسیب در دانشجویان پزشکی سوزن بخیه و سرنگ (6/47 درصد و 22 درصد) و در دانشجویان پرستاری آنژیوکت و سرنگ (6/34 درصد و 6/34 درصد) بود. بی احتیاطی و حواس پرتی عامل اصلی وقوع این آسیب ها بود. انگشت دوم دست چپ و راست بترتیب 31 درصد و 26 درصد آسیب دیده بودند.نتیجه گیریبا توجه به بالا بودن شیوع آسیب در دانشجویان پزشکی و پرستاری، برنامه ریزی های آموزشی دقیق به منظور کاهش خطر آسیب ضروری است. همچنین با توجه به آسیب بیشتر در انگشتان دوم در هر دو دست، مقاوم سازی دستکش ها در انگشتان دوم پیشنهاد می شود.کلیدواژگان: آسیب اجسام تیز، دانشجویان پزشکی، دانشجویان پرستاری

Page 1Background And AimA large number of postpartum women suffer from short and long-term complications of episiotomy. The aim of this study was to compare Chromic Catgut and Vicryl Sutures wound healing in postpartum women.Materials And MethodsThis study was a randomized clinical trial. The sample was 102 pregnant women who were randomly divided into two groups. The episiotomy wound repaired by Chromic Catgut and Vicryl respectively. 4-6 weeks after delivery both groups were evaluated for wound healing. Wound healing was evaluated using the scale (REEDA). The reeda scale consisted of 5 items including redness, edema, ecchymosis, discharge, approximation. The data was analyzed by t-test and MannWhitneyEu test.ResultsThe results showed that the two groups had no significant difference in terms of age, gestational age, parity, duration of the second stage of delivery, the time between the delivery and repair of episiotomy, length of episiotomy repair and education. Page 7Background And AimThe Eastern Mediterranean Regional Office of the World Health Organization has paid attention to blood and blood products safety as a necessary standard in the Patient Safety Friendly Hospitals Initiative (PSFHI). The objective of the present study was to investigate the quality of Haemovigilance system; the quality of blood preparation and patient monitoring at the time of transfusion, after implementation of the patient safety standards.Materials And MethodsThis study was done on a single group with a repeated measure design in Baharloo hospital, Tehran, Iran. The data from 2012 (in the early establishment of standards) was compared with the data from 2014 (i.e. Four years later after implementation of the standards). 100 blood units were selected through a convenient sampling. The sample size was estimated using the Cochranes Formula. The data were collected via a checklist. The data was analyzed descriptive statistics and the paired sample t-test.
Findings: The mean scores of the quality of blood preparations were (6.92±0.299) and patient monitoring (7/96±0/654) after the establishment of patient safety standards, The scores increased in comparison with the early establishment of standards (2.52±0.611, 1.64±0.482). Page 15Background And AimClinical education is one of the principal elements of nursing education. Cultural competency as one of the clinical education outcomes is an important issue in medical and nursing education by it health care providers can gain the ability of working efficiently. Cultural competency can be achieved by implementation and purposeful utilization some learning strategies and processes. The purpose of this article is an overview of utilization of new teaching approaches in clinical education of nursing students and getting information about achieving to learning outcomes, particularly in the cultural competence field.Materials And MethodsThis was a review article. Persian databases, including Magiran, IranMedex, SID and Irandoc were searched using keyword's clinical training, nursing, innovative teaching methods, clinical education. A total of 62 articles were collected, then 26 relevant articles were included in the final analysisResultsAmong related studies, six concept maps, six clinical education related, four varieties of mentorship approaches, three blended clinical teaching, and two computer-based approach were reviewed and the rest were other approaches.ConclusionThe results showed that cultural competency is a missing component in nursing education. Considering the importance and necessity of cultural competency, it is recommended that clinical education approaches particularly innovative strategies should use. The qualitative and quantitative methods should use for assessment and improvement of education of this important component.Keywords: Training, Nursing, Cultural Competency

Page 27Background And AimSharp injuries are very common among health care workers. They make injured worker concern and they need fallow up with diagnostic tests and preventive medications. The aim of this study was to investigate prevalence of sharp injuries, locations and their main causes among medical and nursing students.Materials And MethodsThis was a cross-sectional study. Data was collected by a questionnaire containing demographic characteristics, frequency of sharp injuries, the ward that sharp injuries was occurred, locations of injury and reasons for them. Estimated content validity index and Reliability of questionnaire were 0.831 and 0.786. Data was analyzed by Chi-square test and Fishers exact test using SPSS 16.
Findings: In 41.3% of students sharp injuries had been occurred at least once. The prevalences were 48.4% in medical and 28.3% in nursing students. 66% of medical students and 100% of nursing students had been received preventing information. Most injuries were occurred in the emergency and internal medicine wards. Indiscretions and distraction were the main causes of injuries. The second finger of left and right hand was the most common place of sharp injuries and the injury rate were 31% and 26% respectively.ConclusionThis study showed a high prevalence of sharp injuries among medical and nursing students. Thus, to reduce the sharp injuries, precise educational programs are suggested. According to the results more injuries were happened in the second finger of both hands, strengthening of the gloves in these areas is recommended.Keywords: Sharp Injuries, Medical Students, Nursing Students
